Officials of the Ministry of Healthcare of RF published a new procedure for the provision of medical care for infertility. It says when to use assisted reproductive technologies, the same clearly defined contraindications and limitations to the use of IVF, surrogate motherhood, and more. For example, a woman with HIV or tuberculosis, is no longer able to rely on artificial insemination.
For a start let us explain what includes the concept of “assisted reproductive technology” or ART. In RF this includes methods for the treatment of male and female infertility, in which all or some stages of conception and early fetal development take place outside the mother’s body. This includes not only a well known in vitro fertilization, but also surrogacy, the use of donor gametes and cryopreserved (frozen) embryos. A man and a woman, married officially or de-facto, as well as single women, have a right for the use of assisted reproduction technology.

Until the entry into force of the new order of the Ministry of Healthcare, ART could have been done on your wish. The only limitation was presence of a woman medical contraindications for IVF or other means of artificial insemination. Now the situation has changed. Under the new rules, before such procedures are prescribed to women, doctors should certainly conduct a full examination of her body, to determine the cause of infertility and, try to cure it.

“Before settling for artificial insemination, I always advise my patients to undergo medical examinations and try to cure the cause of infertility – says obstetrician-gynecologist Eugene Martyushova. – However, not all have the patience for it. Infertility treatment is a long process, and not always pleasant, so many women immediately decide to use IVF or other means of artificial conception. Now, women who did not get pregnant will have to undergo treatment for infertility. This would include procedures such as laparoscopic and hysteroscopic correction, stimulation of ovulation, exclusion of male infertility. If, after 12 months, despite the best efforts and treatment, pregnancy does not occur, only then ART will be prescribed.”
However, not all patients will be subjects to such rules and terms. The order stipulates that when a woman is over 35 years, the council of physicians can send her to artificial insemination before 12 months’ period is over.

Besides health officials have compiled a list of restrictions for the IVF. If previously conditioned only by the presence of absolute contraindications, now women with certain genetic diseases such as hemophilia, ichthyosis, and others may be denied the process. Not recommend the use of in vitro fertilization in case of decreasing number of own “dormant” follicles in the ovaries (ovarian reserve). The same obstacle for the procedure may become a state in which it is recommended to use donor’s gametes, embryos and surrogacy. Please, remember that all of the above is not a contraindication to IVF, but only limitation. The final decision on the procedure will be made by a physician.

List of absolute contraindications to applying ART also undergone significant changes. If previously a woman could be denied conducting of these procedures only in the case of her mental illness, tumors, cancer, and congenital anomalies of the female genital organs, but now a long list of diseases was added there.

Among infectious diseases officials identified tuberculosis, viral hepatitis, syphilis and HIV. The list also includes cancer, blood cancer and severe diabetes. It should be particularly noted that these are contraindications to all types of ART. That is now a woman with HIV cannot even find a surrogate mother for her child. All attempts will be illegal. I do not know who and how will assess it, but, in my opinion, such measures are violating women’s rights. Eventually not only drug addicts or prostitutes suffer HIV, but very decent girls, too. And now they are deprived of any opportunity to have children.”
